If the illness is mentioned at all, surely it must indicate that it was the cause of death, otherwise there would be no point mentioning it. You wouldn't say "struck by lightning, after suffering from bunions for years". Would you?

Indeed, this is the conclusion I have come to after "thinking aloud" on this thread.
However, if someone was struck by lightning at a narwhal museum, I think that should be mentioned!
In modern, electronic newsprint, the obit could discretely say that the fellow had died, and you could "highlight the hidden text" or "click here" for the gory details.
