Apparently the astronauts forgot, or were not instructed, to switch the magazine on the camera from a color film to a b&w film. If nobody here can give the answer, you can try to search the Flight Journal for clues about Magazine B, or you can search this 350 page PDF file for clues: Analysis of Apollo 8 photography and visual observations.

One result of the crew being really busy could be that inappropriate exposures were used. I think it was on Apollo 8 that a roll of very high-speed film (ASA 1600?, intended for Earthlight photography) was given an ordinary daylight exposure appropriate to something maybe 16 times less sensitive, and Kodak came up with a processing technique to salvage the images.

Yes if the change in location relative to the Moon between shots provides the proper parallax. Also the images would have to be rotated to provide horizontal alignment for viewing. I looked at them through my red/blue anaglyph glasses and the colors are close enough to work pretty well but I haven't had time to check out the parallax issue.
